Pre-Academy Registration

I, the undersigned, give permission for my child to participate in the Saint Johns Soccer Academy programs, and hereby willfully acknowledge that my signature below attests to my understanding and agreement that:

I understand that playing soccer involves certain risks of physical injuries, such as bruises, sprains, strains, broken bones, and concussions. I acknowledge that my child will be exposed to these risks during practices, games, and other soccer-related activities.

In consideration of my child being allowed to participate in the Saint Johns Soccer Academy, I agree to assume all risks and hazards associated with the sport of soccer. I agree to release, indemnify, and hold harmless the Saint Johns Soccer Academy, its directors, officers, employees, volunteers, agents, and sponsors from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child while participating in the Saint Johns Soccer Academy.

I understand that the Saint Johns Soccer Academy may take photographs, video recordings, or other images of my child during soccer activities. I agree that the Saint Johns Soccer Academy may use these images for promotional or educational purposes.

I certify that my child is in good health and is able to participate in the Saint Johns Soccer Academy. I agree to provide the Saint Johns Soccer Academy with any necessary medical information, including allergies, medications, and medical conditions, that may impact my child’s participation in soccer activities.

I have read and understand this waiver form and agree to all its terms and conditions.
